First, do no harm: Danger in standard treatment for a serious lung disease

ScienceDaily (May 20, 2012) ? A combination of three drugs used worldwide as the standard of care for a serious lung disease puts patients in danger of death or hospitalization, and should not be used together to treat the disease, called idiopathic pulmonary fibrosis, according to the surprising results of a rigorous independent study.

The study, which appeared online May 20 in the New England Journal of Medicine to coincide with a presentation at the annual meeting of the American Thoracic Society, was conducted by IPF Clinical Research Network, funded by the National Heart, Lung, and Blood Institute of the National Institutes of Health.

"The findings show the importance of testing even those treatments that doctors give routinely for any type of condition -- to see if they truly help, and don't harm, patients," says University of Michigan Health System lung specialist Fernando Martinez, M.D., who will present the results.

Martinez and his colleagues report that patients in the mild to moderate stages of the progressive lung-scarring disease had a far higher chance of dying or being hospitalized if they were taking a three-drug combination used worldwide, compared with those taking a placebo.

What's more, the three-drug combo yielded no improvement in lung function, or even slowing of loss of lung function, compared with placebo. Results from a group taking the single drug, N-acetylcysteine (NAC), are still being gathered and analyzed.

This evidence is from a randomized, placebo-controlled, double-blind trial that included patients with a definitive diagnosis of IPF who were treated at 25 centers taking part in the IPF Clinical Research Network or IPFNet. The study was stopped early when an interim analysis showed signs of harm from the three-drug combination of prednisone, azathioprine and NAC.

The findings should cause physicians worldwide to stop using this combination to treat IPF patients similar to those in the trial, say the authors.

And, the dramatic finding of harm from a standard treatment should cause physicians to apply rigorous testing methods to other types of treatment, and highlights the importance of independent federal funding for such studies, says Martinez.

The authors salute the volunteer IPF patients who agreed to be randomly assigned to a treatment or placebo for 60 weeks.

Martinez, an internationally known IPF researcher and clinician in the U-M Medical School's Division of Pulmonary Medicine, remarks that results will soon be known for the group taking NAC alone, compared with those taking placebo. The current paper and presentation do not include results from this group.

In the results presented this week, the authors report that eight patients in the group of 77 assigned to the three-drug combination died, compared with one in the placebo group. A total of 23 of the three-drug patients were hospitalized during the trial, compared with 7 in the placebo group. There was no sign that the three-drug combination slowed the progression of IPF or improved lung function, as measured by forced vital capacity.

The study is called PANTHER-IPF, for Prednisone, Azathioprine, and N-Acetylcysteine: a Study That Evaluates Response in Idiopathic Pulmonary Fibrosis. Except for a donation of NAC and a matched placebo by the company that makes the drug, there was no industry support for the work.

IPF, which affects nearly 100,000 Americans, slowly steals the ability to breathe freely. Its cause or causes are not clear, which is why it is called "idiopathic." Over time it leads to the buildup of scar tissue in the lungs that accumulates in a distinctive honeycomb pattern that can be seen on biopsy or CT scan. It is known as an interstitial lung disease because it affects the tissue around the air sacs in the lungs.

IPF patients live an average of five years after diagnosis, though a lung transplant at a center such as U-M's Transplant Center can extend life for years beyond. Most patients are over the age of 65 when diagnosed, but IPF can strike younger people as well.

Because lung transplants are such a dramatic and rarely available therapy, researchers at U-M and other centers are working to find new treatments while also studying the underlying biological factors in the disease. The PANTHER-IPF trial was designed to test a standard therapy in a rigorous way.

In addition to Martinez, who serves as corresponding author, the research paper was written by Ganesh Raghu, M.D., University of Washington; Kevin J. Anstrom, Ph.D., Duke Clinical Research Institute, Duke University; Talmadge E. King, Jr., M.D., University of California at San Francisco; and Joseph A. Lasky, M.D., Tulane University.

Snacking in America: More May Not Be Better | AICR Blog

Tomatoes and Cheese Snack

One article in today?s Cancer Research Update looks at a study on how specific snack choices may affect the amount you need to eat to be satisfied. While it?s too early to draw conclusions from this study, we do know that our snacking habits are changing and not for the better.

Americans snack more often than we did 35 years ago and we?re eating more calories than in 1977, according to the 2008 National Health and Nutrition Examination Survey (NHANES).

In 1977 Americans reported, on average, eating about 3.9 times a day and now ? about 5.6 times a day. Snacks now account for about 1 out of every 4 calories we eat ? twice as much as 35 years ago.

And, we are eating more ? about 200 calories more every day.

Some of the calorie increase is coming from carbohydrate rich foods. This includes sugar-sweetened beverages often consumed as a snack. Today, for those who drink sugary beverages, the average daily amount is 23 oz., 100 calories more than 35 years ago.

The snacking has not helped us eat more vegetables; in fact the survey shows we are eating fewer vegetables today.

These trends may explain our increasing waistlines because sugary beverages are linked to overweight and obesity and eating more vegetables may help people stay a healthy weight.

In addition, obesity is a major risk factor for many common cancers and a diet high in vegetables is linked with lower risk for several cancers.

Snacking can be a positive part of a healthy diet ? here are some tips to make your snacks count toward health:

  1. Portion out your snack onto a small plate or bowl. If you eat out of the package or nibble every so often, it?s easy to eat more than you need to satisfy your hunger.
  2. Focus on filling most of your snack plate with low calorie foods such as fruit and vegetables. Add a little lean meat, dairy or a few nuts for some protein which can help stave off hunger later, just keep the portions small.
  3. Limit your snacking to times when you are truly hungry and not just bored or looking for a break. If you need a break ? stretch, walk or step outdoors for some fresh air.
  4. Drink more water and other non-calorie beverages like unsweetened tea. Sugary drinks don?t keep you satisfied as long as food does.
  5. Try logging everything you eat using a notebook, an online food tracker or an app. That? can help you be aware of what and how much you?re eating, so you can make choices for health and be smart with indulgences.

Here are recipes for a couple of special vegetable snacks.

What are your go-to snacks that keep you satisfied but don?t weigh you down?

Motherhood | Study Finds Unrealistic Images Of Mothering... | Stuff ...

MICHELLE DUFF

If you don't bond with your baby immediately, you're a bad mother, right?

Wrong ? and it's not compulsory for pregnant women to have a "glow" about them, either.

These common misconceptions are tackled, along with other motherhood myths, by researcher Anita Darrah in her PhD thesis, `The Distressing Case of Modern Mothering', which seeks to pinpoint why so many women struggle after giving birth.

More than 180 women were involved in the study, which found women often held unrealistic pre-pregnancy expectations that could lead to postnatal depression, stress and anxiety when not realised.

Romanticised notions about motherhood were rife, with a "mythical image of mothering and mothers" perpetuated by society and culture.

Ms Darrah, a clinical psychologist whose own battle with postnatal depression inspired her research, found that becoming a mother was traditionally considered a natural, non-problematic event.

While this was true for many, research suggested one in five modern women experienced postnatal depression; an even greater number suffered postnatal anxiety.

Ms Darrah, who is graduating from Massey University in Palmerston North alongside 1325 others this week, said motherhood had changed vastly, as home births ? followed by long periods of family care ? were replaced by clinical, hospital births and "how-to" guidebooks on mothering. The power was taken out of women's hands.

"Rather than a natural phenomenon, pregnancy and childbirth was now a medical event, and it was dangerous and foolhardy for women to undertake such an endeavour without the guidance and direction of experts," Ms Darrah wrote.

In this confusing climate, the clash between modernity, tradition, expectations and reality made motherhood a "distressing" time.

A focus group of 10 women reported feelings of inadequacy after childbirth, with a perceived loss of status and a shattered image of themselves as the ideal mother.

A survey of 175 women aged 18-36 found more than 69 per cent agreed with key myth statements like: "Mothers have an instant bond with their baby," "Having a baby cements a relationship," and "Not bonding quickly can have long-term consequences".

This confirmed that an image of an idealised mother was held by the majority of young women, Ms Darrah said.

"Women in New Zealand struggle with the transition to mothering. They struggle because they are unsupported. They struggle because they are exposed to mythical images of motherhood which set them up to feel failure. They struggle because successive governments have systematically removed support for families and devalued the mothering role."

Women should be supported by health professionals who should discuss expectations and encourage realistic understandings of childbirth, she said.

Along with Ms Darrah and her Massey graduands, more than 2000 students are graduating from Victoria University in Wellington this week.
