KRAS and BRAF mutation screening in metastatic colorectal cancer ...

Researchers report that screening for KRAS and BRAF mutations can reduce the cost of anti-EGFR treatment for metastatic colorectal cancer but with a very small reduction in overall survival according to a new study published on November 28 in the Journal of the National Cancer Institute.

Metastatic colorectal cancer patients whose tumors harbor mutations in KRAS (and to a lesser extent, in BRAF) are unlikely to respond to costly anti-EGFR therapies. Screening of patients who are candidates for these therapies for mutations in one of these genes (KRAS) has been recommended, with the goal of providing treatment to those who are likely to benefit from it while avoiding unnecessary costs and harm to those who are not likely to benefit. However, the real-world impact of mutation screening for both KRAS and BRAF is unclear.

To better understand the impact of mutation screening with regard to health outcomes, costs, and value, Ajay S. Behl, Ph.D., M.B.A., of the HealthPartners Research Foundation in Bloomington, Minnesota, and colleagues, performed a cost-effectiveness analysis that took into account the treatments, resection of metastases, and survival for the different types of metastases. They conducted patient-level decision analytic simulation modeling comparing four strategies involving KRAS and BRAF mutation testing to select treatments for metastatic colorectal cancer patients: no anti-EGFR therapy (best supportive care); anti-EGFR therapy without screening; screening for KRAS mutations only (before providing anti-EGFR therapy); and screening for KRAS and BRAF mutations (before providing anti-EGFR therapy).

The researchers found that compared with no anti-EGFR therapy, screening for both KRAS and BRAF mutations showed a very high (ie, unfavorable) incremental cost-effectiveness ratio, meaning it was very costly in relation to its benefits. Compared with anti-EGFR therapy without screening, screening for KRAS mutations saved approximately $7,500 per patient; adding BRAF mutation screening saved another $1023, with little reduction in expected survival.

The authors write, "In general, our results are less supportive of the use of anti-EGFR therapy than previous analyses, and they indicate lower cost savings from KRAS testing than previously reported. Although we cannot confirm that anti-EGFR therapy is a cost-effective use of health care resources, we can affirm that KRAS testing is cost-saving. BRAF testing may offer additional savings."

In an accompanying editorial, Josh J. Carlson, M.P.H., Ph.D., of the Department of Pharmacy, University of Washington, and Scott D. Ramsey, MD, PhD, of the Division of Public Health Sciences, Fred Hutchinson Cancer Research Center, both in Seattle, note two practical points highlighted by the study: that molecular testing is as much about generating cost savings by identifying nonresponders as it is about improving survival by identifying responders, and that good modeling must account for the fact that community practice (as opposed to clinical trials) "is messy." They write, "most importantly, this study of an unusually accurate test raises important issues that should be considered for other molecular tests in other settings."

Senate Dems divided over cuts to benefit programs

In this Nov. 27, 2012, photo, White House press secretary Jay Carney speaks during his daily news briefing at the White House in Washington. Senate Democrats are deeply divided over whether cuts to popular benefit programs like Medicare and Medicaid should be part of a plan to address the nation?s financial problems, raising a big obstacle to an agreement to avoid the fiscal cliff, even if Republicans agree to raise taxes. Much of the focus during budget negotiations has centered on whether congressional Republicans would agree to raise taxes in exchange for spending cuts. "It is the president's position that when we're talking about a broad, balanced approach to dealing with our fiscal challenges, that that includes dealing with entitlements," Carney said Tuesday. (AP) ? Deep divisions among Senate Democrats over whether cuts to popular benefit programs like Medicare and Medicaid should be part of a plan to slow the government's mushrooming debt pose a big obstacle to a deal for avoiding a potentially economy-crushing "fiscal cliff," even if Republicans agree to raise taxes.

Much of the focus during negotiations seeking an alternative to $671 billion in automatic tax increases and spending cuts beginning in January has centered on whether Republicans would agree to raising taxes on the wealthy. President Barack Obama has insisted repeatedly that tax increases on the wealthy must be part of any deal, even as White House officials concede that government benefit programs will have to be in the package too.

"It is the president's position that when we're talking about a broad, balanced approach to dealing with our fiscal challenges, that that includes dealing with entitlements," White House press secretary Jay Carney said Tuesday.

But even if GOP lawmakers agree to raise taxes, there is no guarantee Democrats can come up with enough votes in the Senate to cut benefit programs ? as Republicans are demanding.

"I hope not if it means Social Security or Medicare benefit cuts," said Sen. Sheldon Whitehouse, D-R.I.

There's a growing consensus among Senate Democrats and the White House that Social Security should be exempt from any deficit-reduction package. But some centrist Democrats in the Senate argue that fellow Democrats must be willing to consider cuts to Medicare and Medicaid in order to get concessions from Republicans on taxes.

"It has to be both ? a significant revenue increase as well as spending cuts," said Sen. Max Baucus, D-Mont., chairman of the Senate Finance Committee.

Sen. Kent Conrad, D-N.D., who is retiring as chairman of the Senate Budget Committee, said rising health care costs in Medicare and Medicaid are helping to drive future spending, making them an essential part of a long-term deficit-reduction package.

"I've been part of every bipartisan group here. We've always put everything on the table," Conrad said. "If you're going to solve this problem, you're going to have to deal with where the spending is and the revenue can be raised."

But senators like Baucus and Conrad increasingly are being drowned out by other Democrats emboldened by the recent election results to fight against benefit cuts.

Sen. Tom Harkin, D-Iowa, said he is willing to find savings in Medicare and Medicaid by making them more efficient. But, he said, he won't support benefit cuts.

"I think the election spoke very strongly about the fact that the vast majority of American people don't want to cut these programs," Harkin said.

Congress and the White House are devoting the next three weeks to finding at least a bridge over the fiscal cliff by reducing the sudden jolt of higher taxes and spending cuts in January while laying a framework for addressing the nation's long-term financial problems next year.

Obama wants to let tax rates rise for wealthy families while sparing middle- and low-income taxpayers. Some Republican leaders, including House Speaker John Boehner of Ohio, have said they were willing to consider making the wealthy pay more by reducing their tax breaks. But most Republicans in Congress adamantly oppose raising anyone's tax rates.

Negotiations are going slowly as each side waits for the other to make concessions.

Democrats already have tried to take Social Security off the table. Carney, the White House spokesman, said Monday that changes to the massive retirement and disability program should be done separately from any plan to reduce the deficit. That's the same position taken by 28 Democratic senators and independent Sen. Bernie Sanders of Vermont in a letter to fellow senators in September.

"We will oppose including Social Security cuts for future or current beneficiaries in any deficit-reduction package," said the letter, which was signed by many top Democrats, including Majority Leader Harry Reid of Nevada. In the House, Democratic leader Nancy Pelosi has taken the same position, not only on Social Security, but also on Medicare and Medicaid.

"There hasn't been the slightest suggestion about what they're going to do about the real problems, and that's entitlements," said Sen. Orrin Hatch of Utah, the top Republican on the Senate Finance Committee. "There's a certain cockiness that I've seen that is really astounding to me since we're basically in the same position we were before" the election.

The No. 2 Senate Democrat, Dick Durbin of Illinois, has tried to find common ground, saying he would be willing to require wealthy seniors to pay more for Medicare benefits. But on Tuesday, he said those discussions shouldn't be part of rushed negotiations to avoid the fiscal cliff.

"If we simply stand by and say 'don't touch Medicare in any way, for any reason, ever' we are inviting a crisis that opponents can exploit to eviscerate Medicare or even to end it," Durbin said. "Progressives should be willing to talk about ways to ensure the long-term viability of Social Security, Medicare and Medicaid, but those conversations should not be part of a plan to avert the fiscal cliff."

Barclays says 5 staff fired over Libor rigging

LONDON (Reuters) - Barclays has fired five employees following its internal investigation of the rigging of Libor interest rates and disciplined another eight people, the head of its investment bank said on Wednesday.

Rich Ricci, chief executive of Barclays' corporate and investment banking, said "a lot" of the individuals identified in its internal probe had left the bank so it could not take action against them. He said authorities had been told the details of those people.

Barclays was fined $450 million (281 million pounds) in June by U.S. and UK regulators after it admitted manipulating the setting of Libor interest rates, a benchmark used to price loans and financial contracts.

The scandal cost the bank's chairman and chief executive their jobs, and prompted Britain to set up an inquiry to assess standards in banking.

"We carried out a very thorough investigation as you will have seen from the reports of the authorities and we have subsequently held an internal review and disciplinary process of those individuals," Ricci told a panel of lawmakers as part of the inquiry.

"Internally when we conducted our process there were 13 people who have been disciplined - of those that remained - and five have been terminated. There's an appeal process for some of the terminations," he said.

Ricci said the bank was in the process of concluding its investigation. It was the first time Barclays has revealed how many staff had been fired or disciplined.

More than a dozen banks are also under investigation by authorities over the setting of Libor rates, but Barclays is the only bank to have settled its case with regulators.

BRANCH COUNTY ? The games are over and the Big Eight All-Conference teams have been chosen. The following is a bit on each first team selection.
Josh Holton, RB, Union City
Holton was the Big Eight Conference MVP and a All-Big Eight First Team pick on offense. Holton showed what he could do as the featured back for the Chargers as he rushed for 1,820 yards and 23 touchdowns on the year. His speed and athleticism helped him average more than a first down per carry (11.59 yards).
"He has the most unique blend of athletic ability, speed and physicality I've ever coached in terms of a running back," said Davis. Holton was also second team defensive pick at defensive back.
Scott Alwood, OL, Union City
Alwood ? a All-Big Eight First Team offensive lineman ?was a huge key to the success of the Charger offense that opened holes for an 1,800 yard back. Alwood was the anchor, making calls on the offense that averaged 286.6 yards per game. "Scott was our best offensive lineman," said Union City coach Chris Davis. "He is extremely intelligent."
Geordan Blowers, DL, Union City
Blowers anchored the Charger defensive line, leading the way with 41 tackles at his position. Blowers was always up to task every Friday night, facing the best offensive lineman on opposing side. "Geo" was recognized for his play with an All-Big Eight First Team pick on defense.
"He is always a presence for us on the defensive line," said Davis.
Chris Rieger, DE, Union City
Rieger ? an All-Big Eight First Team pick on defense ? was a force on the defensive side of the ball for Union City, pulling in an astounding 96 tackles and nine sacks on the season. In addition, Rieger had three interceptions, caused two fumbles and recovered two fumbles.
"We could always count on him to come through with a tackle for loss, sack, or forced fumble," said Davis.
Levi Drumm, LB, Quincy
The senior inside linebacker earned Big Eight All-Conference First Team defense honors as one of the top two tacklers for the Orioles for the 2012 campaign. Drumm recorded 54 tackles and a fumble recovery on the season to help lead the Quincy defense in what turned out to be a challenging season for Quincy.
Second team picks included Quincy's Jimmy Strock (RB) and Connor Bassage (K) and Union City's Devon DiGiacomo at LB.

Human waste helps us look back on history

Human waste can map two millennia of history and climate change in a remote, Arctic settlement in Norway, according to a new study.

The findings suggest that human waste deposits could help researchers untangle the effects of natural and human-caused climate changes.

"We're able to really effectively disentangle what's human and what's natural," said study co-author Robert D'Anjou, a researcher at the University of Massachusetts. "We're able to date the onset of human settlement in the area and also look at agricultural practices and settlement history alongside the changing environment."

Poop through the years
Archaeologists have used traces of ancient feces to re-create the history of specific archaeological sites: For instance, determining whether a latrine was ever used, D'Anjou told LiveScience. But nobody had used human waste to track the arc of human settlement and its effect on the environment. [ Through the Years: A Gallery of the World's Toilets ]

To do so, D'Anjou's team drilled several sediment cores from the bottom of the frigid Lake Liland on Vestvagoya Island, part of an archipelago of Norwegian islands north of the Arctic Circle. Since the Iron Age, people have farmed around this lake, and one of the largest Viking longhouses of the Arctic sits by the lake's frigid waters.

A 9.2-foot (2.8-meter)-long core captured about 7,000 years of time in the region, as well as 1.5-foot (45-centimeter)-long core containing sediments from about 2,300 years to 200 years ago.

The team analyzed coprostanol, a chemical component of human waste, as well as chemicals found in the waste of cows, sheep and other livestock. To tie human population levels to climatic changes, they also measured polycyclic aromatic hydrocarbons (PAH), a fingerprint of burning vegetation.

Levels of human and livestock waste byproducts jumped sharply around 2,300 years ago, when people first settled by the lake. At the same time, levels of PAH rose, probably because the new settlers burned forests to make way for grazing land and farming, D'Anjou said.

But from A.D. 650 to 850, human poop output dropped and the grassland gradually became reforested ? possibly because settlers left the shores of Lake Liland for the newly discovered territory of Iceland.

Another drop in human waste corresponded to the plague's peak during the late 1300s, when about 80 percent of the population moved from these areas or died off, D'Anjou said.

During the Little Ice Age, which lasted from the 1500s to the 1800s, PAH levels increased while the amount of coprostanol remained constant, indicating the settlers were probably burning much more wood to stay warm.

"In the Arctic, it's going to be very cold, so we see an increase in burning wood to keep warm," D'Anjou said.

Odd methods
The new study reveals how the climate and agriculture were intertwined, D'Anjou said.

"Slight variation in the growing season for agriculture greatly influenced the settlement and the population alongside this lake," D'Anjou said.

When the weather turned too cold for farming, settlers may have abandoned their farms for the coasts, where they fished for cod, he added.

But it also reveals a new way to piece together the history of the recent past, he said. Many times, researchers look only for specific compounds in sediments, but that limits their ability to re-create history, D'Anjou said.

"They have blinders on to what could be a really cool story in the strangest of locations," he said. "This one happened to come from poop."
